It is helpful if you provide dates, places, full names of any persons involved and how you feel your complaint could best be resolved. Any documents, such as employment records or medical records, that you think are relevant to the complaint should be attached to the complaint form.
The Commission staff cannot offer opinions on what the outcome of a complaint will be, or if the Complainant will be successful. However, staff can provide information as to what the law states and how it may apply to a given situation.
The Complainant must sign and date the complaint form. Complaints cannot be made anonymously.
When the complaint is filed, Commission staff may contact you to clarify any issues raised in the complaint.
